Appointments are virtual.
Dr. Hazzah will help you select your membership tier and set up automatic monthly payments at your initial appointment.
There is a FOUR-MONTH minimum commitment for Deluxe and Active Membership access and 12 MONTHS for Basic Membership access.
Dr. Hazzah is licensed in IL, PA, TX, NY, NJ & FL
Patients based in the United States must be residents of a licensed state listed above to utilize our Active & Deluxe Membership options.
Out-of-state patients interested in Functional Medicine are invited to enroll in our Basic Membership for virtual learning.
There are no residency restrictions for international patients.
Holistic Medical Pyramid is not accepting Medicare patients at this time.
*One-time initial assessment with a fee of $399.00 is required before enrollment into the membership program.
**The cost of lab testing is not included in the membership fee. Specialty lab testing is paid directly to the lab. Dr. Hazzah will help you determine which tests you need. All patients will not need all tests.
